Delivery & Presentation
- Midline abdominal wall defect COVERED by a translucent membranous sac containing abdominal viscera (usually liver and/or bowel).
- Associated almost universally with severe chromosomal abnormalities in 50% of cases (Trisomy 13, 18, 21, or Beckwith-Wiedemann Syndrome).
- Cord clamping: Ensure clamping is securely far from the sac base to avoid catastrophic injury to herniated bowel contained in the cord base.
NICU Stabilization
- Carefully wrap the intact sac in sterile saline-soaked laparotomy sponges, then cover with a sterile bowel bag or silastic covering to minimize evaporative losses and preserve heat.
- Insert gastric decompression tube (10F Replogle) to continuous low suction (20-40 mmHg).
- Stat bedside Echocardiogram: HIGH rate of concurrent significant congenital heart disease (ToF, VSD, ASD, Coarc).
- Evaluate for hypoglycemia meticulously right after birth (extremely common if Beckwith-Wiedemann suspected - look for macrosomia, macroglossia, ear creases).
Surgical/Medical Management
- Small defects: Early primary surgical closure.
- Giant Omphalocele (liver containing, small abdominal cavity domain): 'Paint and Wait' strategy.
- Paint the sac sequentially with topical agents (e.g., silver sulfadiazine, povidone-iodine, or medi-honey) to allow native skin to granulate and grow over the defect over months.
- Establish secure central venous access (PICC) for long-term parenteral nutrition and prepare for eventual delayed ventral hernia repair at 1-2 years of life.
Diagnostic & Management Algorithm
- Omphalocele Delivery Sequence: [START] Known omphalocele -> [ACTION] Wrap in sterile bowel bag -> [DECISION] Respiratory compromise? -> [YES] Intubate -> [GOAL] Staged closure or paint & wait
