General Surgery Clinical guideline

Gastroschisis Management

Delivery Room Management

  • Ensure prompt delivery room presence of neonatology
  • Place infant's lower half and exposed bowel completely into a sterile bowel bag (drawstring silo)
  • Position infant on right side to avoid tension on mesenteric root (prevent ischemia)
  • Insert a large bore Replogle/orogastric tube (10F) to continuous low suction to decompress stomach

NICU Stabilization

  • Establish robust vascular access (often avoid umbilical lines depending on anatomy/surgeon preference, obtain peripheral/PICC)
  • Fluid management: Tremendous evaporative losses (start IV fluids at 1.5 - 2.0x normal maintenance, ~150 mL/kg/day)
  • Initiate broad-spectrum IV antibiotics (e.g., Ampicillin/Gentamicin)
  • Monitor closely for signs of bowel ischemia (discoloration, increasing acidosis)

Surgical Strategy Focus

  • Primary closure vs. staged reduction using a preformed spring-loaded silo reduction
  • Daily gradual reductions of the silo at the bedside until abdominal domain accommodates the viscera
  • Final operative closure when fully reduced
  • Prolonged TPN required due to significant expected dysmotility post-closure

Perioperative & Wound Protocol

  • Strict optimization of acid-base balance (pH > 7.30) and normothermia (36.5-37.5 C) before surgical transport.
  • Pain Management: Morphine infusion 10-20 mcg/kg/hr or Fentanyl infusion 1-2 mcg/kg/hr; utilize FLACC/NIPS scales.
  • Avoid excessive traction on bowel or silo structures; inspect mesenteric color Q1H for ischemia.
  • Wound care: Sterile saline packing or specialized dressings strictly per Pediatric Surgery direction.
  • Do NOT administer nephrotoxic drugs post-op until urine output is > 1.5 mL/kg/hr.

Standardized Discharge & Follow-Up Criteria

  • Adequate weight gain explicitly > 15-30g/day over a continuous 3-5 day period.
  • Maintenance of temperature in an open crib with standard clothing for 48 hours.
  • Successful completion of the 90-120 minute Infant Car Seat Challenge.
  • Caregiver successfully completed CPR training, rooming-in, and independent medication administration.
  • Referrals placed for High-Risk Infant Follow-Up Clinic, Early Intervention (PT/OT), and appropriate sub-specialists.

Diagnostic & Management Algorithm

  • Gastroschisis Reduction Sequence: [START] Silo placed in Delivery Room -> [DAILY WARD ROUND] Evaluate bowel edema and perfusion -> [DECISION] Is tension low? -> [YES] Squeeze/reduce 1-2 cm of bowel manually and tie silo lower -> [EVALUATE] Monitor Peak Inspiratory Pressures (PIP) and Urine Output -> [GOAL] Fully reduced in 3-7 days, then OR for final facial closure
