Neonatal Thrombosis (Arterial & Venous / RVT)

Risk Factors & Pathogenesis

  • Neonatal hemostasis is uniquely balanced ('developmental hemostasis' - low levels of BOTH procoagulants like Factor II/X and anticoagulants like Protein C/S/Antithrombin).
  • Vascular catheters (UVC, UAC, PICC) are responsible for >80% of all neonatal thrombotic events.
  • Additional precise risk factors: Polycythemia, severe dehydration, maternal diabetes (IDM), perinatal asphyxia.
  • Renal Vein Thrombosis (RVT): Classic presentation in neonates. Triad of macroscopic hematuria, palpable unilateral/bilateral flank mass, and severe consumptive thrombocytopenia (platelets trapped in massive clot).

Diagnostic Imaging Protocol

  • Gold Standard: Targeted Doppler Ultrasound (cranial, abdominal, or extremity depending on catheter site or symptoms).
  • Echocardiogram: Mandatory for suspected Right Atrial thrombosis related to terminal positioning of UVCs or central lines.
  • Aortic Thrombosis (UAC complication): Presents with sudden loss of femoral/pedal pulses, profound lower extremity blanching or cyanosis ('blue toes' or line demarcations). Remove UAC immediately.

Anticoagulation & Thrombolysis (Dosing Guidelines)

  • Low Molecular Weight Heparin (LMWH) - Enoxaparin: Drug of choice. Due to rapid neonatal clearance and increased volume of distribution, neonates require higher doses than adults. Dose: 1.5 - 1.7 mg/kg SQ every 12 hours (for infants < 2 months).
  • Monitoring Enoxaparin: Measure Anti-Xa levels 4 hours post-dose. Target therapeutic range: 0.5 to 1.0 Units/mL.
  • Unfractionated Heparin (UFH): Used for highly unstable patients or anticipated surgery due to rapid reversibility. Loading dose 75 Units/kg, maintenance 20-28 Units/kg/hr. Target anti-Xa 0.35-0.7 U/mL.
  • Thrombolysis (Alteplase / tPA): EXTREME RISK (Brain Hemorrhage). Reserved absolutely for life-, limb-, or organ-threatening massive arterial thrombosis unresponsive to heparin. Dosed precisely via infusion (0.1 to 0.5 mg/kg/hr); strictly monitor fibrinogen.
  • Before tPA, always perform Head Ultrasound to rule out pre-existing IVH.

Diagnostic & Management Algorithm

  • Arterial/Venous Clot Algorithm: [START] Line-associated Thrombus detected -> [DECISION] Life/limb threatening? -> [YES] Consideration for tPA / Surgical thrombectomy -> [NO] Enoxaparin (LMWH) / UFH anticoagulation
