Maternal ITP (Autoimmune Thrombocytopenia)

Maternal ITP vs NAIT Clarification

  • Pathogenesis: Mother has an established autoimmune disorder (ITP or SLE). Maternal IgG autoantibody complex cross the placenta and destroy *both* maternal and fetal platelets.
  • Strict Delineator: Mother will have THROMBOCYTOPENIA (unlike NAIT, where maternal platelets are completely normal).
  • Infant severity: Typically less severe than NAIT. Only ~10% of neonates born to ITP mothers develop platelets < 50k, and rate of catastrophic Intracranial Hemorrhage (ICH) is very low (< 1%).
  • The exact depth of neonatal thrombocytopenia does NOT strictly correlate to the depth of maternal thrombocytopenia or mode of delivery.

Rigorous Monitoring Protocol

  • Obtain an exact cord blood platelet count at delivery.
  • Nadir (lowest point) of platelet count in infant typically occurs between Day 2 and Day 5 of life as maternal reticuloendothelial system mapping clears existing infant platelets.
  • Monitor daily CBC/platelet counts until a steady upward trend is firmly established for 48 consecutive hours.
  • Perform Cranial Ultrasound only if platelets drop < 50,000, or if any neurologic symptoms manifest.

Management Limits

  • Most infants require strictly observation alone.
  • IVIG: Administer 1 g/kg IV (may repeat x1) if platelets drop < 30,000/mcL or if actively bleeding.
  • Platelet Transfusion (Random Donor): Transfuse only if profound clinical bleeding, or if platelets < 20,000/mcL. (Note: Transfused platelets will also be destroyed by the circulating maternal autoantibody, providing only transient support).
  • Recovery: The maternal antibody fully clears the infant's circulation within 4 to 12 weeks, leading to spontaneous complete recovery.

Diagnostic & Management Algorithm

  • Maternal ITP Infant Protocol: [START] Mother with ITP -> [ACTION] Cord blood platelet count -> [DECISION] Plt < 30k or bleeding? -> [YES] IVIG -> [NO] Serial platelet checks days 1-5
