Hematology Clinical guideline

Anemia of Prematurity (Detailed Protocol)

Triple Pathogenic Etiology

  • 1. Impaired Erythropoiesis: Extreme blunting of renal Erythropoietin (EPO) response to tissue hypoxia due to hepatic-to-renal EPO synthesis transition phase.
  • 2. Iatrogenic Phlebotomy: Massive rapid depletion of total RBC volume (neonatal blood volume is 80-90 mL/kg) from constant laboratory testing in the NICU.
  • 3. Shortened RBC Lifespan: Preterm RBCs survive only 40-50 days (compared to 120 days in adults).

Clinical Hemodynamic Effects

  • Often peaks at 4 to 10 weeks of life. Characterized by normocytic, normochromic reticulocytopenic anemia.
  • Symptoms: Increasing absolute frequency of apnea and bradycardia (A/B spells), resting tachycardia (> 160 bpm), bounding precordium, flow murmur.
  • Growth restriction: Significant poor weight gain (< 15 g/day) despite taking 120-140 kcal/kg/day, due to massive increased cardiac work compensating for low oxygen-carrying capacity.

Strict Prevention Tactics

  • Delayed Cord Clamping (DCC) for 30-60 seconds at delivery minimizes the need for early postnatal transfusions by providing a placental transfusion of up to 20-30 mL/kg of whole blood.
  • Umbilical Cord Milking (UCM) can be considered if DCC is contraindicated (e.g. resuscitation needed), but UCM is STRICTLY CONTRAINDICATED in infants < 28 weeks due to extreme risk of IVH.
  • Perform strict micro-draw methods utilizing pediatric minimum-fill laboratory tubes.
  • Utilize point-of-care (POC) testing and non-invasive sensors (transcutaneous bilirubin, continuous multi-wavelength oximetry) to drastically cut phlebotomy.

Transfusion Thresholds & Triggers

  • Strict adherence to 'Liberal vs Restrictive' clinical thresholds based on respiratory status.
  • Intubated / Severe pulmonary disease: Transfuse if Hgb/Hct < 11 g/dL or 35%.
  • Moderate support (CPAP/Nasal Cannula / increasing O2 requirements): Transfuse if Hgb < 9-10 g/dL or 28-30%.
  • Convalescent / Room Air (stable but symptomatic with apnea/poor growth): Transfuse if Hgb < 7-8 g/dL or 20-22%.
  • Pharmacotherapy: Recombinant Erythropoietin (rEPO) is NO LONGER universally recommended in early courses due to association with increasing severity of Retinopathy of Prematurity (ROP).

Diagnostic & Management Algorithm

  • AOP Management: [START] Hgb < 8 + Apnea/Tachycardia -> [ACTION] PRBC Transfusion 10-15 mL/kg -> [MONITOR] Reticulocyte count
