Endocrine Clinical guideline

Neonatal Hypoglycemia

Risk Factors & Presentation

  • Infant of Diabetic Mother (IDM)
  • Large for Gestational Age (LGA) or Small for Gestational Age (SGA)
  • Late preterm infant (34-36 6/7 weeks)
  • Perinatal stress (ischemia, infection, hypothermia)
  • Symptoms: tremors/jitteriness, lethargy, poor feeding, apnea, cyanosis, seizures (often asymptomatic)

Screening & Initial Workup

  • Point of care (POC) glucose screening for at-risk infants (within 30-60 mins of life/post-first feed)
  • Confirm POC glucose < 40-45 mg/dL with a stat serum/plasma glucose level (do not delay treatment while waiting)
  • Further diagnostic workup (critical sample: insulin, cortisol, GH, ketones) if hypoglycemia is severe, persistent (> 48 hrs), or requires high glucose infusion rate (> 8-10 mg/kg/min)

Management Algorithm

  • Asymptomatic & glucose 25-40 mg/dL (first 4 hrs): Feed / breastfeed. Recheck in 1 hr.
  • Symptomatic OR glucose < 25 mg/dL (first 4 hrs) OR < 35 mg/dL (4-24 hrs): IV dextrose bolus (2 mL/kg of D10W) and start IV infusion (D10W at 80 mL/kg/day)
  • Monitor glucose regularly until consistently > 45 mg/dL prior to feeds.
  • Consider continuous glucose monitoring or endocrine consult for sustained refactory cases.

Endocrine & Metabolic Stability

  • Dextrose gel (40%): Massage 200 mg/kg (0.5 mL/kg) thoroughly into buccal mucosa for initial asymptomatic hypoglycemia.
  • D10W Bolus: Exactly 2 mL/kg (yields 200 mg/kg of glucose) over 1-3 minutes if symptomatic.
  • Glucose infusion rate (GIR) calculation: GIR (mg/kg/min) = (IV Rate [mL/hr] × Dextrose %) / (Weight [kg] × 6).
  • Monitor for rebound hypoglycemia 30-60 minutes after discontinuing or weaning IV dextrose.
  • Hold oral feeds if respiratory rate > 70 to prevent aspiration while hypoglycemic.

Standardized Discharge & Follow-Up Criteria

  • Adequate weight gain explicitly > 15-30g/day over a continuous 3-5 day period.
  • Maintenance of temperature in an open crib with standard clothing for 48 hours.
  • Successful completion of the 90-120 minute Infant Car Seat Challenge.
  • Caregiver successfully completed CPR training, rooming-in, and independent medication administration.
  • Referrals placed for High-Risk Infant Follow-Up Clinic, Early Intervention (PT/OT), and appropriate sub-specialists.

Diagnostic & Management Algorithm

  • Hypoglycemia Algorithm: [START] Point of Care Glucose < 45 -> [DECISION] Symptomatic? -> [YES] IV Dextrose bolus (2mL/kg D10) -> [NO] Feed & recheck -> [EVALUATE] If still < 45, start IV Dextrose infusion
